  I chose Add a Contact Form when creating a page and the contact form is showing up on my website with the Name and Email fields pre-filled. I would like them to be blank

  Hi there,

  When you’re signed in to your account at WordPress.com, the contact form pre-fills the fields with your own information. If other WordPress.com users visit the page, they will see their own information filled in there. However, they can always change that information if they want to enter something different. If a visitor isn’t signed in to WordPress.com, they will see a blank form when they visit the page.
