  when I add a contact form to my blog it shows up with my own name, website and email address filled in. (I may have mistakenly done this when initiating my blog, and then somehow saved it).

  I can’t get back to the original blank form to add to my posts. How do I do this?

  The blog I need help with is: (visible only to logged in users)

  Yes, but your visitors don’t see that infformación, only you do.
  Log out from WordPress.com and see the contact form again
