I don’t know what you mean by button. You create a Page and customize your own contact form by following that guide. You can create a custom menu http://en.support.wordpress.com/menus/ and add all the Pages you want to appear into it http://en.support.wordpress.com/menus/#adding-pages so they appear in your horizontal menu bar.

When you are adding or editing a post or a page, you will see the edit screen with the buttons for bold italic alignment and so on. Above that row of buttons you’ll find buttons for “add media” “add poll”
“add contact form”

Is there any way to add a file attachment field and button to a contact form that would e-mail the attachment to my e-mail address?

Contact forms automatically send emails to you when readers click the submit link. That is what they are for ie. private communication with readers off the blog by email. http://en.support.wordpress.com/contact-form/
