  For some reason the contact form brings up my personal information rather than a blank form?

  The blog I need help with is: (visible only to logged in users)

  Hi, I checked your site, and it shows me a blank form.
  The support document for contact form says:

  Once you publish your post, this shortcode will be transformed into a fully functional contact form. Logged-in WordPress.com users will see the contact form pre-filled with their account information:

  No, only you can see that information. To check this, log out and see the contact form again.
