Contact Form

I am having a problem with creating the contact form (add pages) My blog information is populating on the contact form template and I am trying to removed it by adding the page to trash. When I create a new contact form the information is still populating. How can I remove my information from showing on the contact form template
The blog I need help with is: (visible only to logged in users)

There will be autofilled information in a contact form or a comment box but your information is visible to you when you are logged in and have not cleared your browser cache and cookies. No one else can see it.
When someone makes a comment on your blog, unless they have cleared their cookies, WordPress.com will recognize them and complete the details for them. No one else will be able to view those details in comment boxes or in contact forms. The next person who comments will see blank fields. Commenters will not see each others details.
Also know that there is an Admin link on posts and pages that only you as Admin and any editors you have can see when you are logged in. It’s for your convenience and your visitors cannot see it. You cannot remove it as it’s coded into the theme templates and we cannot edit templates or themes.

Log-out – when you are logged in WordPress.com remembers who you are – if I visit your site I will see MY info on your form – if you visit my site you will see YOUR info on my form
