Contact form

    Trying to create a contact form. I can creat the form itself, but it has all of my own details in it. I cannot create the form in a blank state. All of the fields are already filled in with my own username, email, web address etc instead of the user having a blank form to fill in. I have tried editing but it always ends up with my information
