  I added a contact form on my post for customers. I decided to check my website ,when i got to the page where the contact form was located. I was supposed to see empty slots to fill in, but instead I saw all my details in the contact form

  There’s nothing wrong. You’re seeing your own name because you’re still logged in to WordPress. Other visitors won’t see your name in the contact form.
