Try this contact form: http://wordpress.org/extend/plugins/contact-form-7/

It’s very good, the installation guidelines are supplied.

@sheeroko That plug-in would work for a wordpress.ORG install, but this is the wordpress.COM forum, and wordpress.COM blogs cannot use plug-ins.
