  I am trying to make a contact form for my website. It worked but when I go to my website my user name and email address show up as examples. How do I remove this.

  This is a regular misunderstanding by new users. You only see your details as you are still logged in to WordPress.com. No one else sees them. If I visit your site whilst logged in to WordPress.com I see my details pre-filled. Other visitors without a WordPress account will see blank fields.
