  Every time I try to add the contact form it adds my personal information to the form once published. How do I make these fields stay blank?

  The blog I need help with is: (visible only to logged in users)

  Hi there,

  Only you can see your personal info.

  The form is automatically populated with the details associated with a visitors WordPress.com account.

  If you view the form when logged-out, it should be blank.
