  I have deleted my contact form by mistake on the independent publisher theme and now I cannot get It back. Can this be fixed ? As I really don’t want to change the theme .

  Hi there, it looks like the Contact page is still linked in the navigation menu, but is missing the contact form.

  If you just had a basic contact form you can either add it back again or use page revisions to restore an earlier save of your page.

  You need to paste this code while editing html:
