  Is there any way to add images in a contact form? Or to place images to the right of the question on the page?

  Can I have multiple contact forms on a page?

  You cannot add multiple contact forms to a single page or post. They would have to be on separate pages or posts.

  You can add images before or after the block of contact form code that wordpress puts into the page or post, but you cannot put images beside the different labels in the contact form code.

  Thanks…are you saying I can insert an image between questions?

  Or just before or after the entire contact form?

  @Rich: Actually you can add whatever you like if you edit the form in the HTML editor.
