  The contact form is inserting my personal information in the fields.

  The blog I need help with is: (visible only to logged in users)

  Not to worry, that’s just because you are viewing the page as a logged in WordPress.com user. If you log out, or view it from an incognito/private browsing window, you will see blank fields as a regular viewer would.
