Contact form appears with my email and name

  When my contact form appears my name and email are written where the visitor should put his Info.I can’t find what i did wrong…any help?

  The blog I need help with is: (visible only to logged in users)

  When you are logged in, this info will be automatically filled out. Any one else seeing the contact form with either see their own info, or it will be blank.
