Contact form as my stuff. ?

Contact form. I am unclear why my contact form as my username, email address and WP’s website address. If this is an error, how do I remove them. I have tried changing the About Page, but the items return.

Log out of WordPress.COM and the info will go away – WordPress.com remembers who you are when you are logged in and visit a contact form –
