Contact Form Auto Fillling my information

Hello,
This is the generated code for my contact form:
[contact-form subject='Message from website!'][contact-field label='Name' type='name' required='1'/][contact-field label='Email' type='email' required='1'/][contact-field label='Phone' type='url' required='1'/][contact-field label='Preferred Method of Contact' type='select' required='1' options='Email,Phone'/][contact-field label='Message' type='textarea' required='1'/][/contact-form]
When I preview my “Contact” page, my personal information is auto-filled into the slots.
How do I delete this, so the slots are just normal and blank?

You see your personal information auto-filled because you are logged in to WordPress.com. If I look at your site, then I’d see my personal information auto-filled in the contact form.
Try this: log out of your WordPress.com account, then go to http://reflexionsbyrachel.com/ . You will see that the form shows no information.
Only logged in WordPress.com (Gravatar) users can see an auto-filled contact form. And what logged in users see is their own information.

Note to staff: it would be helpful if this information could be added to the support article about the contact form. This question comes up in the forum quite often.
