contact form button not showing up/also allowing people to subscribe to my blog

  I have the theme twenty twelve and i want to add a contact form. I read the forum stuff about pressing a button next to the add media button that SHOULD be an add form button, however mine does not have that. I only have an add media button. please advise.

  also i can’t seem to figure out how to add a “subscribe to our blog” button so when we post people who want to follow us will automatically get the updates.

  The blog I need help with is: (visible only to logged in users)
