    good morning and happy Easter!

    I have just realized that I can use the contact form as a questionnaire form, however it is rather ugly…

    1. how can I remove the bold type in the radio box options?
    2. How do I make the contact form have my theme font?
    3. How do I allow more than one selection in the radio box option?

    1. how can I remove the bold type in the radio box options?

    Add the following to make the text with the radio buttons normal instead of bold.

    .contact-form label.radio {
        font-weight: normal !important;
    }

    2. How do I make the contact form have my theme font?

    I’m seeing the font for the contact form as Open Sans, the same as the body font for your posts and pages. Are you seeing something different?

    3. How do I allow more than one selection in the radio box option?

    You could insert separate occurances of the radio button, but this isn’t optimal since once selected, when there is only one button, you can’t deselect it. You can’t select multiple radio buttons within one insertion in a form.

    I would suggest taking a look at Wufoo, which is an online form service that integrates with WordPress.com pretty easily. Here is our support document on it.
