Contact Form displaying my info!

On my contact form how do I remove my info from the fields…what I mean is when people view the contact form the see my user name in the “name” field and my email in the “email” field…how do I make it so those fields are blank?
The blog I need help with is: (visible only to logged in users)

Hi there. You are seeing your username and email in those fields because they are being “auto-filled” by your web browser, which is trying to be helpful by filling in the fields for you. Since normal viewers won’t have logged into your site, they most likely won’t see anything in the boxes (and they will never see your information there, as you haven’t provided it to their browsers).

Log out of your WordPress account, then look at your contact form: you’ll see the form is blank. If I were to look at your contact form while logged in to my account (or any other wp.com user logged in) then I’d see my own information: name email etc. It’s just an autofil function to make the form easy to use.
Others cannot see your information.
