contact form displays my info

  I added a simple contact for for getting visitors basic information; but, it displays my information in the boxes. I just want them to appear blank. Are there some other forms or is there a way to switch that off?

  The blog I need help with is: (visible only to logged in users)

  You see that information because you are logged into WordPress.com. If you view the page in a private tab you’ll see that logged out visitor do see blank boxes. If other WordPress.com visitors see the form when they are logged in, they see their own contact info.
