Contact Form Doesn't Show Up on Site

Hello,
I set up a contact form (using the default WordPress plug-in, I assume) and it shows up in the edit mode for my page ‘Say Hi’ but when I go to the page the contact form isn’t there.The blog I need help with is: (visible only to logged in users)

Like I said, I did insert the contact form. For some reason it doesn’t show up on my page.

Can you please try inserting the form using shortcodes as described in this support document:

It didn’t work, unfortunately. I copied and pasted the shortcode, clicked “Update” to save my changes, and it still doesn’t show up on my site.

When I switch to the HTML tab in the page editor, I can see the following code:
<div class="hidden form-submission-html"> [contact-form] [contact-field label="Name" type="name" required="true" /] [contact-field label="Email" type="email" placeholder="Your Email Address" required="true" /] [contact-field label="Website" type="url" /] [contact-field label="Comment" type="textarea" required="true" /] [/contact-form] </div>The problem is being caused by a class called ‘hidden’ (in the first line). If you remove ‘hidden’, the form will display correctly.
