Contact Form Fields Already Filled?

    I am trying to add a contact form to one of my pages. While I am able to add the actual form with no problems, once it is published all my contact details are visible in the contact fields?

    How do I add the form so that all fields are blank?

    Log out –

    WordPress.COM remembers who you are and when you are logged in fills out the fields for you – if I visit your site your form will have my contact info in it –
