Contact Form – How to make the lines look blank again

I’m making my little store but I have one problem with the contact form
I’m using Fontfolio theme
As you can see here http://i40.tinypic.com/2di29h4.pngThe second image is how I want it to look like
The first image is how my Contact form looks now:
-When I first created it, I put my name and e-mail, but now everytime I make a new form or try to edit it, it ALWAYS show my name and e-mail, I don’t want that, I just want them blank like the Comment part
-I would like that the Comment box Width was like the Name and e-mail boxes,
I edit it with the nine point arrow in the right bottom corner, but it looks big again when I refresh xDThanks guys hope you can help!

Hi it looks like I have the same problem with Contact Form set up.
It is currently displaying my own name and email address on the form.
How is it possible to eliminate this error. Relates to my website http://www.authenticrefleology.com link to the problem page below. Thanks, Caroline -
Log out of your WordPress.com account, then take a look at your contact form. Voilá: no name or email address.
WordPress keeps track of who is viewing the contact form, and if one is logged in to wp.com, then the name and email address is filled in automatically.
For example, when I look at your contact form, I see my own name and email address. If I log out, then the form would be blank.

@mercurywoman: 1tess is right, when I see your contact form I see my own name and e-mail, so other people see them correctly. :)
