Contact form is displaying half populated

  The contact form is auto filling the Name & Email fields with the blog name and associated email address. How do I get those fields to display as blank (not auto filled) fields?
  Contact form at present is set to private.

  The blog I need help with is: (visible only to logged in users)

  This is perfectly normal. You browser stores that information and auto-completes it when you view a form on any blog. Each user will see their own information there, not yours. A logged-out user will just see a blank form.

  You can check this by logging out, clearing your browser cache, and then viewing the form (though the form will have to be public for you to see it while logged out). After clearing the cache the form will be blank, but once you log in your details will again be auto-comleted when you view it.
