Contact form is displaying my details?

  Hi all
  When I’m adding a Contact form to my blog, it is displaying my name and email address in those fields, even though they are blank when I’m creating the form. I don’t want that public! How do I add the contact form without it displaying my name and email address on it on my blog?

  The blog I need help with is: (visible only to logged in users)

  Because you are logged into your site, your info is automatically added. If you log out, and check the form, you should it that it’s blank.
