Contact form issues

  I need help fixing my contact forms on my “contact me” page. I added the contact form, but when it displays on my blog it includes my login name and my email address in the form. How do I fix this?

  WordPress.COM remembers your info when you are logged in – I can’t see your info on the form and if I visited your site I would see my own info in your form
