Contact form not showing up

Hi, I’m trying to create an order form for t-shirts our nonprofit organization made.

I can edit the form in WordPress just fine, but it doesn’t show up on the page.

The page is http://watertownmusic.org/t-shirts/

this was the html

Did you follow all these instructions? http://en.support.wordpress.com/contact-form/ did you put it in the visual editor or the HTML editor?

Yes, followed those instructions. Put it in the visual editor.

Putting that code in the HTML editor works fine for me. Put it in HTML editor not visual.

I’ve now tried in the HTML editor and it still does not show up. Could this be an issue with the theme? I’m using Twenty Eleven.

Not a theme issue. I’ve tried it to my private blog having 2011 theme. Clear your caches and cookies and try it again. Or can you post the link to that contact form page here?

Contact staff on Tuesday when they’re back. That should work.

Can you remove the table you’ve put in and see if the contact form shows up?

Your contact form has been published. You’ve hidden from view yourself, because you’ve added this to your CSS:
```
.commentsblock {
    display: none;
}
```

I tried it on a new unpublished page with no table, and I still can’t see it.

@justpi, do you see the form? I’m not sure what you mean by saying it’s been published. I don’t see it from any computer, logged out or logged in. I’ll still try removing that from the CSS though, thanks.

I know I must be doing something wrong here; I’m just having trouble figuring out what it is. Thanks for your help.

I’ve already told you that it’s not the fault of the code which is working well on my test blog.
The problem is with your settings. Have you tried it after enabling comments?
