Contact-Form not working as described

  The WP documentation indicates that using
  [contact-form]
  a form will appear with separate boxes for name, email address, etc.
  That is not happening. Instead I get a big empty comment-type box.
  Does anyone know what is going on? I cannot find any support on this problem in any of the support documentation.

  Try logging out and then view the contact form again.

  The reason for this is that since you are already logged in, your name, email and website are prefilled (linked to your username), so there is no need to display it.
