contact form not working correctly

    When the contact form completed, all fields are showing up as requested/checked even though that is not what was checked/requested on the contact form.

    Eg. the WOW! Women on Weights page – the contact form at the end of the page when filled out – whether one or all fields are completed, the email sent to me includes requests for information from all boxes.

    Hi Susan,

    I copied your WOW! Women on Weights contact form and gave it a test run. When I submitted the form without checking the last option, this is what I received through email:

    Name: Jeremey DuVall
    Email: omitted
    Sign me up for WOW!: Fall 2014: WOW! Park 96,Tu & Th 9:15am
    I want more information about WOW!: Yes
    I would like to receive LiveWell Newsletters:

    So, when the last box was not checked, the label (I would like to receive…) was still sent, but it wasn’t marked “Yes” as it would have been had I checked that box. Can you let me know if this is what you’re experiencing as well?

    No, it is not what I was expecting.

    I am expecting to see at the most only
    – one of the WOW! classes checked off
    – The “newsletter” and “information about WOW” only if the box was checked.

    What’s happening is every option is being included in the email, instead of just the one’s checked.
    Does that make sense?

    Hi Susan!

    Thanks for clarifying. First, sorry about the formatting of my previous response. I didn’t realize the forum turned my list into a paragraph. If you take a look, my previous post now contains an accurate format of the email notification I received.

    Currently, the email distinguishes whether or not the newsletter or information about WOW sections were checked by either placing a “Yes” next to them (if they are checked) or leaving them blank (if they aren’t checked). There currently isn’t a way to omit the field from the email notification if the box isn’t checked. Does that make sense?
