Contact Form Problem

  I’ve tried activating the Contact Form using the Add A Custom Form button when creating a page or post, but the form appears in my page or post already populated with my website and e-mail.

  How do I insert a blank contact form into a page or post?

  The blog I need help with is: (visible only to logged in users)

  Your information appears because you are logged into your site. Visitors will see a blank form, or if they are logged into a WP account, their information automatically fills the form. To double check, log out, then look at the post with the embedded form.
