contact form problem

I have followed these instructions
“Easy! Put[contact-form]
in any post or page, and a simple contact form asking for the sender’s name, email address, website and comment will be inserted”
and this is what i get

but there is no form. am I missing something.before you flame me: I have searched the forum and cannot find this problem elsewhere. I find similar issues with the contact form but nothing about it showing up blank…
I placed [contact-form] in the visual tab of the text box, changed the page type to contact form (i am using the blix theme) and i get a blank.

I see a contact form there, and *also* that comments are still allowed.
(The form is that “blank” you see. If you log out you’ll see also fields for email and url) -
It will look a lot better and be less confusing if you edit the page and turn comments off in the “discussion” section at the right of the edit/write page.

ok I see, I was looking for individual field for name email subject etc then a big one for the body, and you are right i was very confused by the comments section.

Ah that make sense, as since they are logged in, the name, email, webpage will already be typed up anyway so there is no reason to show them… However wouldn’t it have been better (and it would have looked like a contact form) where Name, email etc were still there but as a logged in user you were not able to modify them unless you logged out? I think it would look better instead of one big blank box…
