Contact form show my email address?

  I tried adding a contact form, but when I published and viewed the page, my name and email address were displayed. Can everyone see this, or will it disappear when I log out?

  The blog I need help with is: (visible only to logged in users)

  yes it will go away – when you are logged in WordPress.COM remembers some info you have filled out before and fills the form for you – visit my site and you will see your info pre-filled out on the contact form on my site –
