Contact form shows MY name and MY email address

  I installed Jetpack particularly because I wanted to get the contact forms. Unfortunately, when I insert a contact form into my pages the formating is not good. More importantly, the fields for name and email address show MY name as “Admin” and my personal email address. I do not know how to remove these. Here is the short code I am provided:

  That is because you are signed in. Nobody on any other computer can see your information.
