Contact Form – Why is it showing my own user name and e mail?

    I have uploaded a contact form page but it is showing my own user name and e mail address.
    How can I keep the contact form and delete my details to allow users to input their own ( and not see mine)
    I created the page by this method-

    Hi Cathie,

    You don’t need to worry, only you see your own details. WordPress contact forms will automatically fill in the name and email address for users who are already signed in to WordPress, otherwise the boxes will be blank. So when I view your contact form (while signed in) it pre-populates with my name and email to save time should I want to get in touch. Your details are kept private – there’s no security issues you need to be worried about.
