contact form will not load on my contact me page

  Hi
  I’ve followed the written and video explanation and it seems very straight forward but the contact form will not appear. any ideas?

  The blog I need help with is: (visible only to logged in users)

  Are you using Internet Explorer? It doesn’t quite work properly with contact forms. Try Google Chrome.

  I see the form just fine. Have you resolved the issue or is it still invisible to you?
