  Hi! Is it possible to create a contact form and introduce the same contact form to 2 or 3 different pages but when it appers in this pages the contact form has a different tittle? Should I introduce some code for that?

  Yes, the contact form can be customized. You’ll just customize it for each page the way you want it to appear.
