my contact form is using my own name and email address in the space where the reader should write it.

Log out and look again – when you are logged in WordPress.COM knows who you are and fills in some of the blanks for you

When I try to add the contact form to my page, the form itself is blank. There is nothing to add!

Also note that your blog visibility is private. That puts us Volunteers in the unenviable position of trying to help by speculation as we cannot view the blog. -
It appears you have not received my other emails. I will attempt to communicate my problem more clearly:
*The site/blog I need help with is Haus Reno
* The site/blog titled Haus Reno is public.
* I know what to do and how to do it
* Ergo I don’t need links to instructionsNow, the template (or form) that tpyically should open for you to customize upon clicking the correct button to insert “Custom Form” is a blank window. When I say blank I mean a there is no template or form to customize. Literally the window is a white blank square.
I do oplogize for your pain. I had no idea that 1) you helpers were volunteers and 2) I should have identified the site/blog in question, before I attempted to convey my problem.
Perhaps the origin of this problem has to do with my primary blog titled mystical engineer (which is set to private). Perhaps that is why Haus Reno is showing a blank window. Meaning there would be no form to “show” if primary blog is set to private.
I’ll change my m.e. settings to public and try again with Haus Reno.
