  My contact form page when selected appears with pre filled out responses. How can I have these areas not to be pre filled out. For example

  Name
  melvinmitchell66 – This area should be Blank

  Email
  (email visible only to moderators and staff) – This area should be Blank

  Your response would be greatly appreciated
