  On my contact page. I see that the boxes are pre-filled with my name, website, and email address. I would think that we want to leave this blank and have the interested party fill in their information for me to contact them with. How can I keep this blank?

  Hi there!

  To add a contact form, go to My Sites and click on Pages → Add. You can also add a Contact Form to existing Posts or Pages by editing them in the Visual Editor.

  The filled in information in your boxes shows only to you. Visitors who are WordPress.com members will see their own information. And other visitors will see blank fields.
