I am trying to restore the original ‘contact’ page where is asks for an email and name in order to contact the blog admin.
The blog I need help with is: (visible only to logged in users)

Since it appears that you may have modified the existing Contact page, you should be able to restore one of the earlier versions. Here is how to do that: https://en.support.wordpress.com/editors/page-post-revisions/
It might be simpler to just delete the current form on the Contact page and then re-add the default Contact form, which includes the line for email address. Here’s how to do that: https://en.support.wordpress.com/contact-form/

Maybe you can help again? I want to add a few diff pages across the top of the home page for people to comment on and start a discussion.
How do I add the “comments” section to each page I add to the top menu bar of the home page?

About commenting, the discussion/comment box is turned on by default on all WordPressdotcom Posts and Pages and unless you’ve turned it off either globally for your entire site or individually in the discussion module on a post or page, it should appear. https://en.support.wordpress.com/settings/discussion-settings/ I do see an open comment box at the bottom of the General Comments page.
Keep in mind that site navigation should be easy for your visitors. Depending on the number of discussions you intend on creating and adding to the top navigation menu on your site, this could get very messy in the long run. If this is still your preference, you should look at creating a Custom Menu: https://en.support.wordpress.com/menus/

After looking at your site, I hope you don’t mind me making a couple of set up suggestions:
1. Given your concerns, you might prefer to have your site’s Privacy setting set to Private. That keeps it out of public view and search engines. The down side is that you will have to directly invite people by email to access the site and once they receive an invitation they will need to open a WordPressdotcom account. The up side is that you can remove their access to the site if it becomes necessary. Here’s more on that: https://en.support.wordpress.com/settings/privacy-settings/2. You might want to consider using WordPressdotcom’s “forum-like” theme: P2 or P2 Classic rather than Twenty Sixteen. https://wordpress.com/themes/search/P2/ The advantages here are that both posts and comments are always visible on the front page of the site.
