  My contact page is populated with my information, how do I remove it to make those fields blank for the user?

  The blog I need help with is: (visible only to logged in users)

  its perfect, when i enter your blog thers just my information prefilled in.

  If you’re logged in, WordPress will put your info in there automatically, just like it does for blog comments.

  Try viewing the form logged out (or from a secondary browser) and the form should be blank.

  Just adding what the others have said – the contact form fills in the info for the person viewing it. You see your info, I see my info, etc. If someone visits your site and they aren’t signed into WordPress.com, then the fields would be blank :)
