Contact page enters my info in instead of leaving fields blank

Hi, I’m using Baskerville, and loaded the ‘Add Contact Form’ button to my ‘Contact’ page. HTML code looks ok (I think) but when I view the page, it has my name, email and url listed in the fields that should be blank. Please help! Thanks.

Hi,
The fields in the contact form fill in automatically using the name and email of the person visiting the page when they are logged into their WordPress.com account. Since you’re logged in, you see your info. When I visit your page, I see my name and email in those spaces. -
