'contact us' form added, already pre-filled in with name & email

  i’ve added a ‘contact us’ form to a page that i’ve published, when i go to view the live page, the name & email address are already pre-filled with my info. how can i change this?

  That only happens because you are either logged in, or you are logged out and wordpress is seeing the cookie that it placed in your browser. Log out, clear your browser cache and cookies, restart your browser and then take another look while not logged in and you will see they are blank.
