Correcting a Contact Us Form from the Form Builder

  Can you tell me once I add a form from the form builder to a page how I can Edit it? I have deleted the page, tried editing, and recreated the page. The first time I built the form I put in my name in the name box, and my email in the email box when trying to create a contact us page. Now every time I try to add a form that information always come up in those blocks even though I have tried every possible way to delete it.
