  Create a form button/link doesn’t not show up like it says in the tutorial.

  Which tutorial? The internet is full of tutorials that apply only to WordPress.ORG software installs but not to WordPress.COM hosted blogs. Note the differences stated here http://en.support.wordpress.com/com-vs-org/ please.
