Created a contact me widget; however, it appears as a code on the website

Dear Sir/ Madam,
I have recently registered nahalhasty.com on WordPress, and I am in the process of setting up my website. In the contact me page, I have created a “contact me form;”however, when I preview the page prior to updating the change, the contact form appears correctly. However, when I actually update the page and go back to look at it, the following code appears instead.
[contact-form to='(email visible only to moderators and staff)' subject='NAHAL HASTY ENQ'][contact-field label='Your Name' type='name' required='1'/][contact-field label='Your E-mail' type='email' required='1'/][contact-field label='Enquiry' type='textarea' required='1'/][/contact-form]
Please help resolve this query.

Hi Shervin,
Your contact form shortcode needed to be placed in the Text tab, rather than the visual tab. I made the change for you. Please take a look at your page:
http://nahalhasty.com/contact-us/
And also, open the page in the editor to see the code, which is now placed under your Text tab:
