Creating a form for people to fill in information

  My website (www.spbroundup.com) has a widget called “Submit Book Info.” What I would like is for when people click the widget, they are taken to a form that they call fill in with their book’s information. Can someone please tell me how to do this? Thanks.

  Sorry, you need to head over to the WordPress.org forums as that is where self-hosted wordpress blogs using the software from wordpress.ORG are supported.

  These forums are for those hosted here on the wordpress.COM free hosting service and things here work differently.
