With the WordPress.com Business Plan you will have the ability to upload a custom theme and install plugins on your site. This is a new change, but requires the Business upgrade.
Upgrades are all billed annually at the time of purchase, so you’ll be charged the 12 month cost at purchase. All plans are fully refundable up to thirty days if you change your mind.
All of the plan upgrades come with a custom domain credit, so you can use that to register a brand new domain through us or bring one to us that you already own through mapping.

I’m not sure I understand the question completely. There are 2 versions of WordPress. What we do at WordPress.com is different than using the WordPress installation with a hosting provider. This guide outlines those differences: https://en.support.wordpress.com/com-vs-org/
With a self hosted site you are fully responsible for it – where as with WordPress.com we take care of the nitty gritty details behind the scenes.
