  Hi, I am trying to set up ‘appointment page’ which will allow users to make an appointment on my blog. I would like to know if its possible to do it. Here are sample link that I am attaching.

  THanks so much for the help. I have learned good lession from your posts. However, Everytime i try add it into post, it does not happen. Is there any possible thing that maybe I am missing?
