Custom Comments style?

  Our blog requires that commenters provide a full name (http://madisonamps.org/amps-commenting-etiquette/). Is there anyway to get this information to come up with the comment box? We seem to always have to email the commenters to ask for the info and permission to post it.

  You may be able to style that with CSS but if not, perhaps you should put at the bottom of each post a link to your posting etiquette page.
